The August 2026 Compounder Roster: Who's Still Shipping Semaglutide Legally
The 503B comment period closed at the end of June, and the compounder landscape has visibly narrowed. Some pharmacies pivoted to strict personalization documentation. Others quietly stopped shipping. Here's how the roster looks as of August 2026, and how to verify any provider you're considering.
Two Categories Emerged
Since the FDA's 503B enforcement guidance clarified in June, active compounders split into two operational categories. The first — call them "documented-personalization" pharmacies — continue to ship compounded semaglutide but only against prescriptions that document a specific clinical basis for the compounded formulation (typically dosing personalization, an excipient allergy, or a combination product). The second — "brand-adjacent" services — pivoted toward facilitating brand-name Wegovy/Zepbound prescriptions instead of compounded product, keeping the telehealth intake but changing the fulfillment.
A smaller third category — pharmacies that continue shipping compounded product without clear personalization documentation — is drawing regulatory attention and should be approached with caution. If a service's own marketing doesn't reference the specific clinical basis for compounding, that's a signal to ask direct questions before enrolling.

What to Ask Any Compounder in August 2026
Five questions separate operationally sound compounders from ones you shouldn't build your treatment on:
- What clinical basis does your prescriber document for the compounded formulation? A good answer names specific reasons (e.g., dose personalization outside brand strengths, excipient-related need). A vague answer is a red flag.
- Are you 503A or 503B registered, and in what state? Both models are legitimate but have different oversight. Verify state pharmacy board registration independently.
- What's your API source and what testing do you perform? Sound compounders name their API source category, cite Certificate of Analysis testing, and offer to share redacted CoAs on request.
- What's your current lead time and interruption history? Ask specifically about the last three months. Any pharmacy currently on 3+ week backorder or with recent multi-week interruptions is unstable enough to require a backup plan.
- What happens if you receive an FDA letter or state action? A prepared pharmacy has a patient-continuity plan and will describe it. If they can't answer, they don't have one.

The Brand-Name Fallback
If the compounded roster looks thin in your state, brand-name pathways via manufacturer direct programs have gotten notably cheaper in 2026. Wegovy's NovoCare cash rate has stabilized in the $199 intro / $349 ongoing range for injectable, with the pill format landing in a $149–$299 tier. Zepbound's LillyDirect vials sit in a $299–$449 range depending on dose. These aren't compounded prices, but they're a lot closer to compounded than they were in 2024.

How to Verify Any Compounder Independently
Every state has a pharmacy board with an online license lookup. Search the pharmacy's legal name (not its marketing brand), verify the license number matches what the pharmacy advertises, and check for any disciplinary actions on the record. The National Association of Boards of Pharmacy (NABP) also maintains a searchable directory of compliant online pharmacies via its ".pharmacy" verified list.
For 503B outsourcing facilities, the FDA maintains a public registration list. If a compounder claims 503B status, it should appear on that list. If it doesn't, treat the claim as unverified.
