What "Cheap" Should and Shouldn't Mean

503A licensure. The pharmacy must be licensed in its home state and (ideally) the state where you reside. Ask for the pharmacy license number and verify it with the state board.
Third-party testing. Reputable pharmacies test each batch for potency, sterility, and endotoxin levels. Ask whether results are available — some pharmacies post certificates of analysis (COAs) on request.
PCAB accreditation. The Pharmacy Compounding Accreditation Board (PCAB) accreditation is the gold standard. Only about 68 pharmacies nationwide have earned it. It's not required, but it signals a higher quality commitment.
Prescriber involvement. A legitimate provider will require a telehealth consultation before prescribing. If a platform lets you order semaglutide without speaking to a provider, that's a red flag — not a convenience feature.
Price Trend: July 2026 vs. Six Months Ago
Average compounded semaglutide pricing has increased approximately 10–15% since January 2026. This reflects two factors: the exit of the cheapest (often least compliant) providers under FDA pressure, and increased compliance costs for remaining pharmacies investing in quality testing and proper documentation. The market is smaller but safer.
Meanwhile, brand-name alternatives have dropped: Wegovy pill launched at $149/month for the starting dose, Foundayo entered at $149/month, and TrumpRx offers brand semaglutide injections starting at $199/month. The price gap between compounded and brand has narrowed significantly — which is exactly what Novo Nordisk and Eli Lilly intended.
