Compounded oral semaglutide — available as sublingual drops, orally dissolving tablets (ODTs), troches, and standard tablets — represents a growing segment of the compounded GLP-1 market. For patients who want to avoid injections, these formats offer a needle-free alternative. But oral peptide delivery introduces quality control challenges that don't exist for injectable formulations, and the testing that ensures accuracy and potency looks different.
Why Oral Peptide Delivery Is Hard
Peptides like semaglutide are chains of amino acids — the same molecules your digestive system is designed to break apart. When you swallow a peptide, gastric acid and digestive enzymes begin degrading it immediately. Brand-name oral Ozempic (Rybelsus) solves this with SNAC (sodium N-[8-(2-hydroxybenzoyl)amino]caprylate), a proprietary absorption enhancer that took years of development and clinical testing to optimize.
Compounded oral semaglutide formulations use different strategies to deliver the peptide past the digestive barrier:
- Sublingual drops and troches: Designed to be absorbed through the mucous membranes under the tongue or in the cheek, bypassing the GI tract entirely. The peptide enters the bloodstream directly through the oral mucosa without passing through the stomach.
- Orally dissolving tablets (ODTs): Dissolve on the tongue and aim for mucosal absorption, similar to sublingual delivery but in a solid dosage form.
- Standard oral tablets: Must survive the stomach and be absorbed in the intestine. These formulations typically include protective coatings or absorption enhancers to improve bioavailability.

Quality Testing for Oral Compounded Peptides
Content Uniformity
For tablets and ODTs, content uniformity testing verifies that each individual tablet contains the correct amount of semaglutide — not just the batch average. This is critical because inconsistent mixing (poor homogeneity) can produce tablets where some contain significantly more or less drug than the label states.
USP <905> defines the content uniformity test: individual tablets are assayed, and the results must fall within a specified range of the label claim. For compounded tablets, this test is more important than for liquids because powder blending introduces more uniformity variability than solution mixing.
Dissolution Testing
For oral tablets intended for GI absorption, dissolution testing measures how quickly and completely the tablet releases its drug content in simulated gastric or intestinal fluid. A tablet that doesn't dissolve properly won't deliver its drug content, regardless of how accurately it was compounded.
For sublingual and ODT formats, disintegration testing (how quickly the tablet falls apart in the mouth) may be more relevant than dissolution. The goal is rapid disintegration to maximize mucosal contact time.
Potency Assay
The same HPLC potency testing used for injectable formulations applies to oral forms — measuring the actual semaglutide concentration against the labeled claim. For liquid drops, this is straightforward. For tablets and troches, the sample must be extracted from the solid matrix before analysis, which adds a preparation step that must be validated to ensure complete recovery of the peptide.
Stability Under Storage Conditions
Oral formulations may have different stability profiles than injectable solutions. Solid dosage forms are generally more stable (less susceptible to hydrolysis) but can be affected by moisture exposure. Sublingual drops in solution face the same degradation pathways as injectable preparations. Stability testing should cover the complete beyond-use dating period under the labeled storage conditions.

Bioavailability studies — measuring blood levels after oral administration — are the gold standard for confirming that an oral peptide formulation actually works. These studies are expensive and rarely performed for individual compounded formulations. This means the clinical effectiveness of compounded oral semaglutide rests more on pharmacological plausibility and patient-reported outcomes than on rigorous pharmacokinetic data.
Sublingual vs. Swallowed: Different Quality Targets
A sublingual formulation that works through mucosal absorption has fundamentally different quality targets than a swallowed tablet:
- Sublingual: Must dissolve quickly, maintain peptide stability in the oral mucosa pH environment, and achieve adequate contact time with absorptive tissue. Testing focuses on disintegration time, pH of the formulation, and mucosal compatibility.
- Swallowed tablet: Must protect the peptide through the stomach, release it in the intestine, and include absorption enhancers to overcome the GI barrier. Testing focuses on acid resistance, dissolution in intestinal fluid, and absorption enhancer activity.
Pharmacies that compound oral semaglutide should be able to articulate which absorption pathway their formulation targets and what testing they perform to validate that approach.
