Medicare GLP-1 Bridge Starts July 1: What Compounding Pharmacy Patients Should Know

Starting July 1, 2026, Medicare will cover FDA-approved GLP-1 weight loss medications at $50/month through the Medicare GLP-1 Bridge program. For Medicare-eligible patients currently paying $99–$300/month out of pocket for compounded GLP-1s, this changes the equation significantly.

What's Covered

• Wegovy (semaglutide injection and tablet)
• Zepbound (tirzepatide KwikPen)
• Foundayo (orforglipron pill — new April 2026)

What's NOT covered: Compounded GLP-1 medications of any kind. Ozempic and Mounjaro (diabetes indications). Any non-FDA-approved formulation.

Should You Switch From Compounded to Brand?

Switch if:

• You're on Medicare Part D and qualify (BMI 35+, or BMI 27+ with comorbidity)
• $50/month is less than your current compounded cost
• You'd prefer the regulatory certainty of FDA-approved medication
• You want access to tirzepatide (Zepbound) — often more effective than semaglutide

Keep compounded if:

• You don't qualify for Medicare or the Bridge program
• Your compounded medication includes personalized ingredients (B12, L-carnitine) you value
• You're on a custom dose not available in commercial products
• Your compounded cost is already under $50/month (rare but possible with some plans)

How to Make the Transition

1. This month: Talk to your doctor. Get prior authorization paperwork started before July 1.
2. Confirm eligibility: BMI threshold + qualifying condition (hypertension, heart failure, CKD, pre-diabetes, OSA, cardiovascular history)
3. Choose medication: Wegovy injection for maximum efficacy, Foundayo for pill convenience, Zepbound for dual-agonist approach
4. Overlap your supply: Don't cancel compounded until your first brand-name fill arrives
