Eli Lilly's once-daily pill is the first small-molecule GLP-1 receptor agonist approved for weight loss. Unlike semaglutide, which is a large peptide molecule, orforglipron is a small molecule — meaning it's easier to manufacture, more stable at room temperature, and doesn't require the special absorption-enhancing technology that semaglutide pills need. In clinical trials, patients lost an average of 12.4% of body weight at 72 weeks.

Ozempic tablets — launched May 4, 2026

Novo Nordisk's oral semaglutide is available in 1.5mg, 4mg, and 9mg doses at over 70,000 US pharmacies. Self-pay pricing starts at $149/month through NovoCare Pharmacy. It's the same active ingredient as injectable Ozempic, just in pill form. For eligible commercially insured patients, copays can be as low as $25 through Novo Nordisk's savings card.

What's Coming Next

The pipeline is full of oral GLP-1 candidates. AstraZeneca's elecoglipron showed positive Phase IIb results at ADA 2026 and is entering Phase III trials. Pfizer, Amgen, and multiple biotech companies are developing their own oral GLP-1s. By 2028, patients may have five or more oral GLP-1 options — driving prices down and quality up through competition.
