The FDA approved the Wegovy pill on December 22, 2025, making it the first oral GLP-1 medication specifically indicated for weight loss. It's the same active ingredient — semaglutide — as the injectable version, just in a daily tablet instead of a weekly injection.

It's available now at CVS, Costco, Walgreens, and through telehealth platforms like Ro, GoodRx, and Hims & Hers. Here's what you need to know about how it works, what it costs, and how it stacks up against the alternatives.

How the Wegovy Pill Works

Oral Wegovy uses the same semaglutide molecule as the injection, but at different doses. The pill dose ladder runs 1.5mg → 4mg → 9mg → 25mg (taken daily), compared to the injectable ladder that tops out at 2.4mg weekly (or 7.2mg for the newer HD version).

The higher oral doses compensate for lower bioavailability — your body absorbs less semaglutide from a pill than from an injection. Despite the different numbers, the clinical effect is comparable.

⚠️ Important: The Wegovy pill requires a 30-minute fasting window after taking it. You take it first thing in the morning with a small sip of water, then wait at least 30 minutes before eating, drinking, or taking other medications. This is a meaningful lifestyle constraint that some patients find difficult.

Novo Nordisk also announced in February 2026 that the list price for all Wegovy and Ozempic products will drop to $675/month effective January 1, 2027 — a 50% reduction for Wegovy. However, this does not affect the self-pay DTC pricing listed above.

The pill is about 1.4 percentage points less effective than the injection in clinical trials, but eliminates needles and refrigeration. For patients whose primary barrier is needle anxiety or cold chain logistics, the tradeoff is worth it. For patients who want maximum weight loss, the injectable edge still matters.

For patients who value FDA approval and want to avoid injections, the Wegovy pill is now the clear choice. For patients where cost is the primary driver, compounded options from verified providers still offer savings at the lower dose tiers.

Who Should Consider the Wegovy Pill?

The oral formulation makes sense for patients who have needle anxiety or injection fatigue, those who travel frequently and want to skip cold-chain logistics, patients whose insurance now covers oral Wegovy (many commercial plans added it quickly), and Medicare beneficiaries who will gain access at $50/mo through the GLP-1 Bridge starting July 1, 2026.

It's less ideal for patients who can't commit to the 30-minute morning fasting window, or for those who want maximum weight loss (the injectable still has a slight clinical edge).
