Zepbound for Sleep Apnea: The First FDA-Approved Drug for OSA
Market snapshot: In December 2024, the FDA approved Zepbound (tirzepatide) as the first and only prescription drug for moderate-to-severe obstructive sleep apnea in adults with obesity. In SURMOUNT-OSA trials, up to half of patients had their sleep apnea effectively resolved after one year of treatment, with an average 18% body weight reduction.
Obstructive sleep apnea affects an estimated 39 million adults in the United States. Until December 2024, there was no FDA-approved medication for it — only CPAP machines, oral appliances, and surgery. Zepbound changed that.
What the SURMOUNT-OSA Trials Showed
The FDA approval was based on the SURMOUNT-OSA program — two parallel trials studying tirzepatide in adults with moderate-to-severe OSA and obesity:
Study 1: Patients who were unable or unwilling to use CPAP therapy.
Study 2: Patients who were already on CPAP and planned to continue.

The sleep apnea approval is clinically significant because it gives the condition its own FDA-approved pharmacological treatment for the first time. It also has major insurance implications — plans that exclude weight-loss drugs may still cover Zepbound when prescribed specifically for OSA.
Why the Insurance Angle Matters
Many insurance plans exclude GLP-1 medications when prescribed for weight loss. However, Zepbound's OSA indication creates a separate coverage pathway:
- Medicare Part D covers Zepbound when prescribed for sleep apnea (a medical condition), not weight management
- Commercial insurers that exclude obesity drugs may cover Zepbound for the OSA indication
- The Medicare GLP-1 Bridge (starting July 2026) also covers Zepbound KwikPen at $50/mo
If you have sleep apnea and obesity, talk to your provider about getting Zepbound prescribed under the OSA indication specifically — it may open coverage that the weight-loss indication doesn't.
Can Compounded Tirzepatide Treat Sleep Apnea?
The same molecule — tirzepatide — is what's in both brand-name Zepbound and compounded versions. However, the FDA approval and insurance coverage pathway are tied specifically to the Zepbound brand. Compounded tirzepatide cannot be prescribed "for sleep apnea" with the same insurance leverage.
Additionally, the regulatory environment for compounded tirzepatide has tightened significantly since the FDA declared the shortage resolved in October 2024. Patients using compounded tirzepatide for any reason face increasing supply uncertainty.
What About Semaglutide for Sleep Apnea?
While Wegovy (semaglutide) does not have a specific FDA approval for sleep apnea, weight loss from any GLP-1 medication can improve OSA symptoms. The clinical data is strongest for tirzepatide because Eli Lilly specifically ran the SURMOUNT-OSA trials to pursue the indication.
For patients already on semaglutide who also have sleep apnea, the weight loss benefits may still help — but the insurance coverage advantage of the OSA-specific indication only applies to Zepbound.
The Bigger Picture: GLP-1s Beyond Weight Loss
The sleep apnea approval represents a trend: GLP-1 medications are rapidly accumulating FDA approvals beyond their original weight-loss and diabetes indications. Wegovy now has approvals for cardiovascular risk reduction (2024) and MASH/fatty liver disease (August 2025). Zepbound has its OSA approval. The pipeline includes trials for heart failure, chronic kidney disease, and addiction.
Each new indication expands the potential insurance coverage pathways and strengthens the clinical case for these medications as multi-system treatments, not just weight-loss drugs.
