The short version
- Compounded semaglutide is still sold, but only under the narrow 503A patient-specific pathway. The shortage exception ended in 2025, and the Fifth Circuit upheld FDA's decision in September 2026.
- The lowest all-in listed starting price we saw was GobyMeds at $299 for a 3-month starter bundle ($99/month), with consultation, shipping and an injection kit included.
- BiltRx lists $81/month, but its page says consultation and shipping are charged separately, so the real cost is higher than the sticker.
- Brand-name Wegovy now has self-pay prices of $149 to $299/month for the pill and a $199 intro price on starting pen doses through Dec. 31, 2026, which narrows the gap.
- Prices below are what each provider's site showed on Oct. 29, 2026. Confirm at checkout before you pay.
As of October 29, 2026, the cheapest compounded semaglutide with everything bundled in is about $99 a month on a prepaid three-month starter plan, and the lowest advertised sticker is $81 a month before add-on fees. Before comparing those numbers, it helps to know what kind of product you are buying now, because the legal ground under compounded semaglutide is narrower than it was a year ago.
Where compounded semaglutide stands legally right now
The short answer: it is legal only in limited circumstances, and the broad shortage-era version of the market is gone.
FDA removed semaglutide from its drug shortage list in early 2025. State-licensed 503A pharmacies lost their shortage-based grace period on April 22, 2025. Since then, a 503A pharmacy can compound semaglutide only for an individual patient when the prescription is not "essentially a copy" of Wegovy or Ozempic. In practice that means a prescriber documents a reason the approved product doesn't fit, such as an allergy to an inactive ingredient.
The compounding trade group Outsourcing Facilities Association sued to reverse the delisting. It lost in district court, and the Fifth Circuit affirmed FDA in the semaglutide appeal (No. 25-10758), with a revised opinion filed September 1, 2026. Law firm Clark Hill's summary of the rulings notes that compounders can no longer lean on the old shortage to justify routine large-scale production.
Separately, FDA proposed on April 30, 2026 to keep semaglutide, tirzepatide and liraglutide off the 503B bulks list, which would block large outsourcing facilities from compounding them from bulk powder. The comment period closed July 30, 2026. When we checked the Federal Register on October 29, the latest notice in that docket was the June comment-period extension. No final decision had been published.
FDA's patient page on unapproved GLP-1s, current as of October 1, 2026, says it knows of no lawful basis for compounding with semaglutide sodium or semaglutide acetate, which are different salt forms from the ingredient in approved drugs. It also recommends discarding a multi-dose vial 28 days after first use, even if medication remains.
That same FDA page counts 990 adverse event reports tied to compounded semaglutide as of May 31, 2026, and notes those reports are likely an undercount. None of this makes every compounded prescription unsafe. It does mean the pharmacy behind your vial matters more than the price.
November 2026 compounded semaglutide prices, side by side
We copied each figure from the provider's own website on October 29, 2026. Where a page showed conflicting numbers, we say so instead of picking the friendliest one. "Monthly equivalent" is the provider's own per-month figure, not our math, unless noted.

How to read the cheapest numbers
The $81 BiltRx figure is a medication price, not a total. Its semaglutide page says consultation and shipping are billed separately and doesn't list those amounts. Get the full checkout total before comparing it with a bundled plan.
HealthRX's $97 requires buying a year upfront. That's $1,163 for a 48-week supply. Its monthly plan starts at $99 and renews at $165. Subscribe-and-save plans renew automatically until you cancel by email.
GobyMeds' $99 is for first-time patients on a slow-titration starter plan. After that, its three-month bundles are priced by dose: $299 for maintenance at 0.2 to 0.4 mg, $399 for plans reaching 0.6 to 1.5 mg, and $499 for 1.9 to 2.3 mg. The site still displays a "Jumpstart" promo dated through December 31, 2025, so we used the standard prices.
Lowest bundled price we verified
GobyMeds' 3-month starter bundle lists $299 with consultation, shipping and an injection kit included. Code x7X72r takes $25 off a first order.

Price competition has shifted from compounders undercutting each other to compounders competing with brand-name self-pay. Novo Nordisk's own Wegovy savings page now lists the Wegovy pill at $149 a month for the 1.5 mg dose, $199 for 4 mg, and $299 for 9 mg and 25 mg. The Wegovy pen has a $199 introductory price for the 0.25 mg and 0.5 mg doses, good for two monthly fills through December 31, 2026, and $349 a month after that. Wegovy HD at 7.2 mg lists at $399.
Novo's page also says pricing will be updated after December 31, 2026. If you're deciding between compounded and brand for 2027, that date is worth watching.
The other trend: more compounded offers bundle additives like vitamin B12 or glycine into the vial. NewSelf's semaglutide is sold that way. Ask your prescriber why an additive is in your formula and what it adds clinically, since an added ingredient is one way a compounded product can differ from the approved drug.
Is the cheapest option the right one?
Price is the easiest number to compare and the least useful on its own. Four things matter more.
Who actually compounds it
The telehealth brand is rarely the pharmacy. Ask for the dispensing pharmacy's legal name and state license number, then look it up on your state board of pharmacy's site. If the company won't tell you, treat that as a reason to walk away.
What form of semaglutide it is
Ask whether the product uses semaglutide base or a salt form. FDA has said sodium and acetate salt forms have no lawful basis in compounding. A legitimate pharmacy can answer this in one sentence.
Whether the dose pricing matches your plan
Flat "any dose, same price" plans (Wellorithm, NewSelf, Gala) protect you as your dose rises. Tiered plans (GobyMeds) are cheapest at low doses and climb as you titrate. If your prescriber expects you to reach maintenance doses, price the plan you'll be on in month four, not month one.
How hard it is to stop
Auto-renewing prepaid plans can lock in money you can't easily recover. Read the cancellation and refund terms before buying anything longer than a month. GobyMeds, for example, says prescriptions are final sale once they ship.
A five-minute checklist before you order
- Get the full checkout total. Include consult fees, shipping, supplies and the renewal price, not just the first month.
- Ask for the pharmacy's name and license. Verify it on your state board of pharmacy site and check FDA's warning letter database.
- Confirm the ingredient. Semaglutide base, not sodium or acetate salt, and ask what any added vitamins or amino acids are for.
- Ask how the prescriber documents need. Under 503A, the prescription is supposed to reflect a clinical reason you need a compounded version. A provider that can't explain this is cutting corners.
- Compare against brand self-pay. If the Wegovy pill at $149 to $299 fits your situation, the compounded savings may be smaller than you think.
Dose changes, missed doses and switching between products are decisions for your prescriber, especially because compounded vials can come in different concentrations than branded pens. Get emergency care for severe, ongoing pain in the upper or middle stomach that may spread to your back, which can signal pancreatitis, and contact your prescriber about persistent vomiting or signs of dehydration.
Frequently asked questions
Is compounded semaglutide legal in November 2026?
Yes, in a narrow form. A state-licensed 503A pharmacy can compound it for an individual patient when the prescription isn't essentially a copy of an approved product. The shortage-based exception that allowed mass compounding ended in 2025, and the Fifth Circuit upheld FDA's decision in 2026.
Did FDA ban compounded semaglutide?
Not outright. FDA proposed in April 2026 to keep semaglutide off the 503B bulks list, which would stop large outsourcing facilities from compounding it from bulk powder. As of October 29, 2026, no final decision had been published in the Federal Register.
Why do some providers show two different prices for the same plan?
Usually it's a mix of introductory pricing, multi-month prepay discounts and older promo banners left on the page. When a site shows conflicting numbers, trust the total on the final checkout screen and screenshot it.
Is the Wegovy pill cheaper than compounded semaglutide now?
At the 1.5 mg dose, Novo lists the pill at $149 a month self-pay, which is close to many compounded plans. Higher pill doses list at $199 to $299. Whether it suits you depends on your dose and your prescriber's view, since the pill and injection aren't dosed the same way.
Can I use a GobyMeds code with other offers?
The code x7X72r is a first-order $25 discount. Check GobyMeds' checkout to see whether it stacks with any bundle pricing shown at the time you order.
